well after a massive amount oof planetary F8-uppage (floods, rain, destruction, quakes) it could have become inactive. think of hawaii. most of it is a combination of active and inactive volcanoes that turn into perfect environments for splendid flora to abound.

As many people have mentioned already, the Gorons have said that Bomb Flowers only grow at Death Mountain and around it.
Both locations of Forest Haven and Dragon Roost Island in Wind Waker seem to match the locations of where Death Mountain and the Lost Woods/Kokiri Forest are located in Ocarina of Time just slightly out north and south. And Bomb Flowers could have grown in Forest Haven due to climate change (yes, this could have happened in the game as well as everything is higher up)
We don't see all of Death Mountain in OOT. Death Mountain keeps going north way past what we have access to. For example, the much further north Zora Domain in TP, is still part of the Death Mountain range.

The presence of bomb flowers is hardly enough evidence to proove Forest Haven is Death Mountain.
Do you not look at other evidence?
Forest Haven is a FOREST. Dragon Roost Island is a MOUNTAIN.
Forest Haven has water and trees. Dragon Roost Island has no trees, and has lava.
Forest Haven has the Great Deku Tree. So did Kokiri Forest.
