Lelouch said that the only people who should kill are those who are prepared to be killed. He lived up to that. Besides, he was prepared to lose his life when he thought Nunally was dead.

Being prepared to be killed and making yourself a target does not mean you have to sacrifice yourself. Really, it makes more logical sense (and thus would have been more likely for Lelouch) to keep oneself alive as a focus for peoples' hatred - as long as they're hating you, they won't hate other people. If you die, they have no reason to hate you any more, only a reason to look back and think "that guy was really evil". Adolf Hitler being dead doesn't stop us from fighting, for one RL example.

But hitler wasn't the only one to hate at that time, and even Hitler had followers. Lelouch didn't have any followers and the hatred of the whole world was focused on him. When he died, that hatred died with him.

That one is confusing to me too. I think it had to do something with believing in Ragnarok. CC not dying was a surprise to them, because she had believed in it too, but she eventually realized that Charles and Marianne were selfish and stopped believing that Ragnarok was good.
