Ha like one would even need to ask such a question. We are talking about the one and only read shirt blue jean suspender wearing turtle stomping princess saving jack of all trades master. Of course he can be a pulmber and a docter at the same time. He is so good that he is a plumber full time and he only needs to be a doctor part time.

It's a game.
Of course Mario can have 2 jobs. By the way, he is also a baseball player, a football player, a party maniac etc. So I really can't find a reason not to be a doctor and a plumber at the same time.
