It is actually not known precisely how, its mere speculation. I can only assume the seal that kept Ganondorf within the Sacred Realm failed to trap him forever. It may have some relevance to the fact that Ganondorf still had the Triforce of Power after he was sent to the Sacred Realm. Apart from that its clearly unknown, so we can only speculate.

Okey, WW is after TP, and TP is after OoT, I think. Since Ganondorf has the triforce of power, he could have broke the seal with it's power. And I don't think you could figure this problem out by playing WW over and over. I hoped I helped.

they always seem to have that happen, like something really strong occurs (like ganon being locked away) and then some how he always gets out, but its never really explained. there are alot of theroys, and i personally dont think about it, cuz its just something the gamer isnt really supossed to worry about, but it is kind off a silly little thing....... also i think hylian kid is rgiht, because well find out how he got out, but ganon wont be the same main villian that he used to be (this could bring lots of questyions and ideas) but im sure when we find out we'll all be suprised or think its cool, or something like that!

In the WW into, it speaks of a darkness, which was followed by Ganon. I beleive that the darkness may just be a metaphor for the ________ _____ (see spoiler)

Regardless, Agahnim had some contact with Ganon prior to releasing the seal, which means that the seal was already weakened or compromised in some way. Of course, that could just mean that Agahnim used one of the natural portals to the Sacred Realm/Dark World, like the ones you use before you gain the Master Sword.
