The game could start in Hyrule with some sort of confrontation that causes Link to doubt himself. Then he is either knocked unconscious or falls asleep and is in a dream-world, but unlike Link's Awakening, it has a much darker feel to it.
Perhaps instead of the usual 8 dungeons, he could have 8 various feats he has to accomplish, each one being a triumph over his doubts. Each one could give him a piece of the Triforce of Courage [though the ToC isn't real of course, it's in his dream] and at the climax of the game, the ToC is reunited and Link awakes with his courage renewed and defeats whatever it was that caused it to doubt in the first place.
I can't think of anything in particular to be the 8 feats, but perhaps it could do with the elements. A high mountain, a deep sea, etc. Each one ending in a boss that represents what he's conquering.
The last boss in the dream world could be Dark Link to appease the fan boys and to represent his final task as being able to defeat all parts of himself that hold him back

That actually makes sense...
hm................................................ ...............................
I suppose we can look at the elements and then choose a pschological issue associated to it... But I'd rather do it the Other way around....
Psychological issue...
1) Fear-Not Going into something due to the fact you believe you will not like the outcome; Jumping off cliffs-Perhaps they add in a new gameplay element that lets you avoid falling damage by Executing a well timed roll (
Parkour
-Alternate Version: Being Sandwiched in between two Rocks-Look for places where the rocks won't hurt you and Hookshot to it
-Alternate Version-Have a choice to go into a dark Cavern with or without a torch. If you choose without, Dark Link will come out and fight you, if you choose with. There will be no Dark Link. Either way, you must defeat Dark Link to get the Key and use the Torch to make your way through the maze.
2) Anger/Happyness/Dissapointment-In the Middle of a room there is a heart Peice, You must fight Wave after Wave of enemies. After each wave, another Heartpeice is placed in the middle. If you ever get a heart Peice in the middle you stop earning Heart Peices and the waves never end. To finish this Quest, you must complete 8/10/whatever waves of enemies without getting a heart peice. (which takes Great Patience, but the point that is being made is that true strength doesn't come from heart peices... it comes from skill)
3) Distraction-The room has one main puzzle where you win by doing sidequests and using them appropiately
-Each Side Quest gives you a power/weapon and changes the main puzzle a bit.
-Gain each power in the right order and use them on the main puzzle to win.
-Nayru's Love is one of the things necessary.... thus if you don't use nayru's love in time... the puzzle must be started all over again
4) Out of Ideas.......
Come to think of it... My Ideas kinda feel like they have a lot to do with the three parts of the Triforce.... Courage, Power and Wisdom....
I didn't even notice until after I was done.....
Thats kinda cute....

They could add in other magics such as telepathy or telekinesis. Of course, for the moment everything would probably make use of the Wii motion sensing. Perhaps there's a puzzle that you have to read the minds of the imps in the room to get a correct rune sequence, then use the moving power of your brain to rearange tiles on the floor, all while avoiding getting noticed as you sneak about and hide in the shadows?

Interesting...
Sounds like new Telepathy enhancing minigames....
I kinda Dig it... But I would rather do it for Rupees rather then main quest...
Just because Telepathy is sooooooooooooo haaaaaaaaaaaaaard...................

And at the end of the game, it becomes questionable wether Link really went on an epic journey to destroy this land or if that was just a dream he had when he became imprisoned there.
